Community Health Equity Specialist

We are seeking a dedicated and compassionate Community Health Equity Specialist to join our team and
focus on asthma remediation and home visits in the historically redlined community of South Stockton.
As a Community Health Equity Specialist, you will play a crucial role in improving asthma outcomes and
addressing environmental triggers by conducting home visits, providing education, and coordinating
remediation efforts for the D.A.W.N. program. This position requires a deep understanding of the
historical context, strong interpersonal skills, and a commitment to promoting health equity in
underserved communities.
